CMT2009 Conference Shirts
Attractive and marine-coloured (turquoise) golf shirts are available for purchase by delegates. The shirts are high-quality golf-shirt style,
and feature the CMT2009 logo embroidered on the front. Shirts are only available for purchase on advance order.

Costs & Payment
The CMT2009 Golf Shirts are available at a total cost of R100.00 each. If you wish to order a CMT2009 Shirt, you can request these on the Shirt Order Form.
Please note that payment with order is preferred, using the credit card details on the form.
Registration Information
A completed registration form from all participants (including speakers) is essential.
Fulltime registration (Super Early Bird, Early Bird, Regular) includes: attendance
at all conference sessions, morning/afternoon teas and lunches, welcome reception,
conference pack, and an electronic copy (CD) of the conference proceedings.
Day registration includes: attendance at the conference sessions, morning/afternoon
teas and lunch on the selected day, a ticket to the welcome reception (irrespective of the
selected conference day), conference pack and an electronic copy (CD) of the conference proceedings.
Student registration includes: attendance at all conference sessions,
morning/afternoon teas and lunches, welcome reception,
conference pack, and an electronic copy (CD) of the conference proceedings.
Students must provide proof of fulltime student status from their institution.
Payment of Fees
Payment of fees must accompany the registration form. Your space will only be secure
once payment has been received. Major credit cards are accepted.
Acknowledgement of Registrations
Registrations received will be acknowledged in writing (e-mail), with confirmation of your
requirements according to your registration form. Delegates must advise the organisers
immediately of any amendments.
Welcome Reception
Delegates of the CMT2009 are welcome to join us for a Welcome Reception at 7pm on 23 June,
at the Vodacom Amphitheater at The Boardwalk. Attendance at the Welcome Reception is included in the conference registration fee. Additional tickets for partners can
be bought at ZAR180.00 / US$30.00 each.
Conference Banquet
The CMT2009 Conference Banquet will be held in the conference venue, the Boardwalk Tsitsikamma Conference Center. The conference
banquet is an optional extra, and not included in the conference registration fee.
Dress Code: Smart Casual
Cancellations and Refunds
Registration Fees - Cancellation of registration must be advised in writing to the
Conference Secretariat. Cancellations made before 30 April 2009 incur a penalty fee of 50%.
Refunds of fees for cancellations after 30 April 2009 will only be made in exceptional circumstances.
Transfer of registration to another person may be made without penalty at any time.
Please note: Cancellations made in the two weeks prior to the conference will not be
processed until after the conference period.
If for reasons beyond the control of the
organisers, the conference were to be cancelled, fees less expenses
incurred will be refunded.
Social Functions - Refunds for social functions will be given, in accordance with the provisions of cancellation of registration.
No refunds will be given for social functions that are cancelled by delegates or partners on
or after the first registration day (23 June), or missed during the conference.
